All applicable documents must be submitted to the sport medicine department 3 days prior to the planned tryouts day. Incomplete packages will not be accepted.
Items required
- Attach a copy of the front or back of health insurance card.
- Signed acknowledgement of risk form.
- Completed insurance or emergency form of Albany state university.
- A completed physical form signed by a doctor.
- All physical needs to be within the last 12 months.
N: B If prospective student athlete is under age 18, please ensure parent or guardian signature where required
ACKNOWLEGEMENT OF WARNING OR RISK
By signing below, you are acknowledged about potential risk injury of varying temporary permanent disability to any of body system at any point during the workout.
ACKNOWLEGMENT OF PHYSICAL
- For you to participate in Albany university tryout you must have undergone a physical administered by a physician.
- For your participation to be accepted your physical must have been accepted by your high school in practice during your senior year in high school.
- You must have received the physical within the past six months
- By signing this form, you acknowledge that you did in fact receive a physical that meets this stipulation.
TWO YEAR OR FOUR-YEAR TRANSFER
- For you to participate in Albany university tryout you must have undergone a physical administered by a physician.
- The physical must have been accepted by your collegiate institution for your participation in athletic.
- You must have received the physical within the past six months or within six months prior.
- By signing this form, you acknowledge that you did in fact receive a physical that meets this stipulation.
